Cooperative Production has 33 residential homes and 5 Day Programs. Our 24/7 supervised residential programs focus on a person-centered planning approach and working with individuals to create homes adapted to meet individual needs. ABI RESIDENTIAL COORDINATOR
QUALIFICATIONS
• High School degree required; college degree preferred
• Ability to comprehend and write English at a traditional Grade 12 level
• Possession of a valid driver’s license, a safe driving record, and an agreement to use one’s personal vehicle for business purposes.
• Acceptable Criminal Offender Record Information (CORI) and fingerprint check
• Three years of management experience working with people with developmental disabilities required
ESSENTIAL DUTIES
• Develop and implement procedures for assessing peoples’ strengths, interests, competencies, etc., with provisions for ongoing reviews of these procedures to ensure relevancy and a dignified process
• Ensure the provision of support services (family needs, transportation, staffing, and technical assistance) as appropriate
• Serve as a liaison between the Agency residential services and other service providers
• Communicate with families/guardians as appropriate
• Implement staff evaluation policy and procedures
• Provide in-service training for assigned topics
• Encourage/require professional development of residential staff
• Develop specific and schedule feedback opportunities for staff
• Provide direct supervision to Community Living Supervisors as necessary, and oversee their supervision of residential staff

• Coordinate licensing and certification requirements of assigned programs
• Ensure homes are properly maintained
• Ensure adequate procedures and protection for the administration of the program and resident finances

• Ensure all participant medical and psychological needs are addressed
• Monitor medication administration procedures and staff recertification training

GENERAL EXPECTATIONS
The CLINCAL SPECIALIST is responsible for participating in the development, monitoring, and implementation of all psychological services, as designated by the Director of Clinical Services, which are designed to enrich the lives of individuals with developmental and other disabilities. This position is specifically designed as being cross departmental collaboration. The schedule is designed to best meet the needs of individuals across settings. This may include specific hours as apart of the residential /day program staffing pattern.
Duties shall be conducted in conformance to the Agency’s purpose and philosophy and in a manner that recognizes values associated with Life Quality.
DIRECT SUPERVISOR: Clinical Director
QUALIFICATIONS
1. G.E.D. Required; Bachelor’s degree strongly preferred
2. Ability to comprehend and write English at a traditional Grade 12 level.
3. Physically capable of performing direct service duties designated in “Note B” under “Essential Duties”
4. Possession of a valid driver’s license, a safe driving record, and an agreement to use one’s personal vehicle for business purposes if so required.
5. Three years of experience collaborating with people with developmental disabilities or acquired brain injury, or a Bachelor’s degree in a related field with one year of experience.
6. Acceptable Criminal Offender Record Information (CORI) and fingerprint check.

ESSENTIAL DUTIES
1. Assist in the development, implementation, maintenance, and evaluation of behavioral programs.
2. Conduct ongoing observations for the purpose of assisting Clinical Director in completing functional assessments, determining appropriate implementation procedures in conformance with Positive Behavior Support, evaluating program efficacy and assisting with staff training.
3. Assist and train staff in implementation of all behavioral/therapy programs and crisis intervention procedures.
4. Assist in ongoing training of staff through Behavior Review Meetings, focusing on positive behavior and support strategies to achieve success.
5. Summarize relevant behavioral data for assigned individuals for all review purposes.
6. Assume supervision responsibility for Program staff relative to implementation of positive behavior support/therapy programs to include training, and corrective feedback.
7. Assist in the development of Individual Service Plans, and in the completion of service documentation requirements, as assigned by the Clinical Director including, but not limited to, assessments, service agreements, progress notes, quarterly, and semi-annual reviews.
8. Participate in medication/psychiatric review process through data summaries, anecdotal reporting, and attendance at meetings, as assigned by Clinical Director.
13. Actively participate in on-going evaluation of appropriateness of individual community placements and make recommendations, as necessary.
14. Assume Instructor and Direct support duties as necessary, and when assigned by the Clinical Director.
15. Keep Clinical Director informed of programmatic and personnel issues as they relate to delivery of Psychology services.
Notes: A. Duties pertaining to participant assistance shall be performed in a dignified and supportive manner so as not to diminish a person’s identity or self-esteem.
B. Required physical assistance includes but is not limited to: transferring people of adult weight ranges to and from wheelchairs, or other adaptive equipment; utilizing physical restraints with individuals who
demonstrate aggressive or self-injurious behaviors and demonstrating CPR skills on the floor, which is a requirement for certification. Staff are required to participate in and successfully satisfy the physical requirements of Safety-Care training (staff need a normal degree of flexibility, the ability to stand, walk, run, and shuffle, transition from standing to kneeling and back, and firmly grasp and raise their hands above their head).
OTHER DUTIES
1. Assume advocacy role as necessary for legal, civil, and human rights and for participation in community and social opportunities.
2. Evaluate appropriateness of community placements relative to work, recreation or other social opportunities.
3. Look for opportunities to develop natural supports with intent of minimizing paid staff supervision.
4. Where appropriate, assist in orienting people to new assignments utilizing existing “orientation program” of host company or organization.
5. Assist other staff with participants, whenever necessary.

CLINICAL ASSISTANT

ABI COMMUNITY LIVING SUPERVISOR II/LPN
The Community Living Supervisor II/LPN is responsible for supervising and implementing an array of services to enrich the lives of people with varied disabilities. Services include but are not limited to, case management, transportation, the development of social relationships, and providing support in the areas of recreational, therapeutic, and personal living skills.
DIRECT SUPERVISOR
Assistant Director of Specialized Services/Residential Coordinator II
QUALIFICATIONS
• Current Licensure by the Commonwealth of Massachusetts as a Practical Nurse
• Ability to comprehend and write English at the traditional Grade 12 level
• Three years supervisory experience working with people with disabilities, ABI experience preferred but not required
• Possession of a valid driver’s license, a safe driving record, and an agreement to use one’s personal vehicle for business purposes
• Drivers must be 19 years of age and completed all required training
• Drivers must meet physical capabilities - required assistance includes transferring people of adult weight ranges to and from wheelchairs, or other adaptive equipment
• Drivers must have a Registry of Motor Vehicles record on file prior to contact with any participants. This is required to be updated on an annual basis
• Acceptable Criminal Offender Record Information (CORI) and fingerprint check
ESSENTIAL DUTIES
• Oversee the day-to-day operation of the program (staff scheduling, medical appointments, community activities, maintenance, vehicles, menu planning, etc.)
• Trains, supervises and evaluates residential staff in conjunction with the ADSS/Residential Coordinator II.
• Supervise, support, and maintain the safety of individuals.
• Provide relevant training or assistance to individuals as identified in their ISP and/or as needed in life routines.
• Specific “training” may be in: communication skills, personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP.
• Monitor and implement service plans throughout the day including behavioral and therapeutic requirements
• Provide case management responsibilities and/or service documentation requirements as assigned and defined by the ADSS/Residential Coordinator II.
• Attend required In-services.
• Assist individuals with medical appointments as necessary.
• Monitor individual’s medical needs as well as doctor’s orders, medication sheets, diets, etc.
• Administer and document medication in accordance with applicable regulations.
• Ensure required documentation is completed including but not limited to Progress Notes; program data; medication; cleaning checklist; Daily Log/Staff Communication; finances; time slips, etc.
• Where necessary, serve as a communication link between residents, family members, guardians and other service providers.
• Assist in the management of the program budget and financial record keeping to ensure accuracy and compliance.
• Assume various other and related duties as assigned by the ADSS/Residential Coordinator II.
LPN (ABI) ESSENTIAL DUTIES
• Confer with physicians, dentists, and therapists as appropriate.
• Respond to participant complaints of illness and recommend appropriate action.
• Confer with pharmacists regarding medications and their acquisitions as appropriate.
• Monitor seizure disorder records.
• Administer minor first aid when necessary.
• Complete all health-related documentation requirements.
• Provide standard First Aid/CPR certification training and/or other assigned trainings to agency employees.
PEER SUPPORT ESSENTIAL DUTIES (If Applicable)
• Provide instructional supports to enhance the skills of the participant to function in the community.
• Provide support, self-advocacy, skills training, independence, and family leadership.
• Other duties as assigned.

CERTIFIED NURSING ASSISTANT DIRECT CARE SUPPORT - ABI
The CNA/ABI is responsible for providing an array of services to enrich the lives of people with Acquired Brain Injury (ABI). Various levels of support are provided in a number of areas, including but not limited to personal care, life routines, finances, health care and clinical services, recreational and social interests, developing relationships, and connecting to the community.
All duties shall be performed in a dignified and supportive manner so as not to diminish a person's self-identity or self-esteem. The ideal candidate will be experienced in providing diverse care to those who need it while remaining patient and calm.
DIRECT SUPERVISOR:
Residential Coordinator and/or Community Living Supervisor
A Sampling of Essential Duties:
• Enhance individuals' lives by monitoring needs and offering personalized assistance and support with activities of daily living to improve quality of life.
• Assess and report any issues to other healthcare personnel.
• Supervise, support, and maintain the safety of assigned individuals.

• Attend meetings, as required, to facilitate participant progress, Agency growth, or general communication among staff and for other agencies.
• Provide training, support and assistance as necessary to aid people in adapting to and participating in community environments.
• Attend all required training and maintain required certifications.
• Attend work on a regular, consistent basis.
• Provide or facilitate transportation arrangements reinforcing the use of natural supports when possible.
• Perform all duties in conformance to, or in recognition of, existing Agency philosophy, policies, and procedures.
• Assist individuals with medical appointments as necessary.
• Administer and document medication per DDS regulations.
• Ensure all required documentation is completed, including, but not limited to, progress notes, program data, medication, cleaning checklist, daily log/staff communication, finances, and time slips.
• Other duties as assigned.
REQUIREMENTS
• Active MA CNA certification program
• High School Diploma or GED preferred
• Ability to comprehend and write English at traditional Grade 12 level.
• Physical capabilities required to perform direct service duties - required assistance may include transferring people of adult weight ranges to and from wheelchairs, or other adaptive equipment
• Possession of a valid driver's license, a safe driving record, and an agreement to use one's personal vehicle for business purposes if so required
• Experience working with people with developmental disabilities preferred

• Monitor seizure disorder records
• Monitor and assess the general health status of participants, including hygiene issues and skin integrity

• Provide standard First Aid/CPR certification training and other assigned training to Agency employees

ABI COMMUNITY LIVING SUPERVISOR
The Community Living Supervisor is responsible for supervising and implementing an array of services to enrich the lives of people with varied disabilities. Services include but are not limited to, case management, transportation, the development of social relationships, and providing support in the areas of recreational, therapeutic, and personal living skills.
QUALIFICATIONS
• High School degree required, college degree preferred.
• Ability to comprehend and write English at the traditional Grade 12 level.
• Possession of a valid driver’s license, a safe driving record, and an agreement to use one’s personal vehicle for business purposes.
• Acceptable Criminal Offender Record Information (CORI) and fingerprint check
• Three years of experience working with people with disabilities
ESSENTIAL DUTIES
• Oversee the day-to-day operation of the program (staff scheduling, medical appointments, community activities, maintenance, vehicles, menu planning, etc.)
• Supervise, support, and maintain the safety of individuals.
• Provide relevant training or assistance to individuals as identified in their ISP and/or as needed in life routines. Specific “training” may be in: communication skills, personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP.
• Facilitate monthly staff and house meetings, in conjunction with the C.O.C.L./Residential Coordinator II.
• Provide counseling and assistance as necessary to aid people in adapting to a new workplace or other community environments.
• Ensure required documentation is completed including but not limited to Progress Notes; program data; medication; cleaning checklist; Daily Log/Staff Communication; finances; time slips, etc.
• Assist individuals with medical appointments as necessary
Schedule: Tuesday through Friday 12 pm -8 pm and Saturday 10 am-6 pm

Services include but are not limited to, case management, transportation, the development of social relationships, and providing support in the areas of recreational, therapeutic, and personal living skills.
Duties shall be carried out in conformance to the Agency’s purpose and philosophy and in a manner that recognizes values associated with Life Quality.
This position is scheduled Saturday 9:00 am - 5:00 pm, and Tuesday through Friday 1:00 pm - 9:00 pm.
QUALIFICATIONS
• High School degree required, college degree preferred.
• Ability to comprehend and write English at the traditional Grade 12 level.
• Possession of a valid driver’s license and an agreement to use one’s personal vehicle for business purposes.
• Three years of management experience working with people with developmental disabilities.
• Acceptable Criminal Offender Record Information (CORI) and Fingerprint check.
ESSENTIAL DUTIES
• Oversee the day-to-day operation of the program (staff scheduling, medical appointments, community activities, maintenance, vehicles, menu planning, etc.
• Trains, supervises, and evaluates residential staff in conjunction with the C.O.C.L./Residential Coordinator II. Participates in interviewing and hiring new staff.
• Supervise, support, and maintain the safety of individuals.
• Provide relevant training or assistance to individuals as identified in their ISP and/or as needed in life routines. Specific “training” may be in: communication skills, personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP.
• Monitor and implement service plans throughout the day including behavioral and therapeutic requirements.
• Provide case management responsibilities and/or service documentation requirements as assigned and defined by the C.O.C.L./Residential Coordinator II.
• Facilitate monthly staff and house meetings.
• Provide counseling and assistance as necessary to aid people in adapting to a new workplace or other community environments.
• Attend required In-services.
• Provide or facilitate transportation arrangements optimally using natural supports.
• Perform all duties in conformance to, or in recognition of existing Agency policies and procedures including the personnel Manual.
• Assist individuals with medical appointments as necessary.
• Monitor individual’s medical needs as well as doctor’s orders, medication sheets, diets, etc.
• Ensure required documentation is completed including but not limited to Progress Notes; program data; medication; cleaning checklist; Daily Log/Staff Communication; finances; time slips, etc.
• Assume various other and related duties as assigned by the C.O.C.L./Residential Coordinator II.
• Where necessary, serve as a communication link between residents, family members, guardians, and other service providers.
• Assist in the management of the program budget and financial record keeping to ensure accuracy and compliance.
WORKING CONDITIONS
Employees are required to assist participants who may demonstrate aggressive or self-injurious behavior through direct intervention and conformance with behavioral programs.
Employee assignments and work schedules are subject to change contingent upon the needs of individual participants or groups of participants.
