Role Summary
The REGISTERED NURSE, DAY HABILITATION, administers, coordinates, and monitors health care services for individuals participating in Cooperative Production’s day services, including day habilitation, community-based day supports, employment services, and brain injury day services. This position helps ensure that medical supports are delivered in a respectful, person-centered manner that fits naturally into each individual’s daily routine.

The Registered Nurse works closely with individuals, families, residential teams, day program staff, health care providers, pharmacists, and human service professionals. This role provides nursing oversight across day program locations, supports regulatory compliance, participates in interdisciplinary planning, supervises assigned LPN staff, responds to health-related needs, and serves as a key resource to staff and program teams.

QUALIFICATIONS
• Current licensure by the Commonwealth of Massachusetts as a Registered Nurse.
• Nursing education commensurate with degree and licensure requirements.
• At least one year of experience in a community health care setting.
• Experience supporting people with developmental disabilities preferred.
• Ability to perform required direct support duties, including health and safety interventions as needed.
• Valid driver’s license, safe driving record, and willingness to use a personal vehicle for business purposes when required.
• Acceptable Criminal Offender Record Information, CORI, and fingerprint check.
• Ability to maintain confidentiality, follow policies and procedures, and communicate professionally with individuals, families, staff, and external providers.

ESSENTIAL DUTIES
• Provide nursing oversight for Cooperative Production day services, including travel to day program sites as needed.
• Supervise assigned LPN staff working in day service locations in collaboration with the Director of Health Services and Human Resources.
• Coordinate or provide nursing coverage for call-outs at day service locations.
• Participate in pre-admission evaluations, interdisciplinary team reviews, ISP meetings, provider meetings, quarterly meetings, annual meetings, and other assigned planning meetings.
• Ensure each individual has a complete medical history and that medical records are accurate, current, and maintained in compliance with applicable local, state, and federal requirements.
• Prepare for and participate in surveys and certifications, including CARF and DDS reviews.
• Review health needs, document nursing notes, track responses to medical interventions, and update annual recertification orders.
• Recommend and follow up on lab work, x-rays, EEGs, and other diagnostic testing as appropriate.
• Document and update immunization information as needed.
• Communicate health needs, plans of care, and relevant updates to day program staff, residential staff, nurses, and other team members across shifts.
• Coordinate medical services with residential nursing staff and serve as a resource to residential programs as needed.
• Consult with physicians, dentists, audiologists, therapists, program coordinators, and other health care professionals regarding treatment plans, progress, and support needs.
• Identify, assess, report, document, and help prevent communicable diseases and infections through appropriate protective measures.
• Respond to emergency situations as needed and complete required documentation.
• Provide health-related instruction to individuals when applicable, including safety, first aid, health and nutrition, personal hygiene, and exercise.
• Participate in staff training on assigned topics, including bloodborne pathogens, diet textures, wound management, and related health and safety topics.
• Become Safety Care certified and participate in behavior management supports as needed.
• Become a backup MAP trainer and complete MAP training at least once per year.
• Advocate for individuals’ legal, civil, and human rights and support participation in community and social opportunities.
• Help identify opportunities to develop natural supports and promote independence.
• Assist other staff with participant support whenever necessary.
• Report any suspected mistreatment of individuals to the Disabled Persons Protection Commission as required.
• Perform additional duties as assigned.

WORKING CONDITIONS
• Assignments and schedules may change based on the needs of individuals or groups receiving services.

ROLE DESCRIPTION
This is a full-time on-site role for a Training Coordinator located in North Dighton, MA. Reporting to the Director of Quality Assurance, the Training Coordinator will be responsible for participating in the development, monitoring, and implementation of all agency trainings, which are designed to provide our staff with ongoing education relevant to the field. Duties shall be carried out in conformance with the Agency’s purpose and philosophy and in a manner that recognizes values associated with Life Quality.

Qualifications

The ABI Nurse Coordinator is responsible for supporting and coordinating an array of services to enhance the health and well-being of individuals with acquired brain injuries (ABI) and other disabilities. This includes overseeing daily medical needs, coordinating care across programs, facilitating transitions between healthcare settings, and ensuring the delivery of services in a manner that promotes dignity, independence, and quality of life. The ABI Nurse Coordinator plays a critical role in maintaining communication between program staff, families, guardians, and medical professionals to ensure each individual’s needs are met.

QUALIFICATIONS
• Current licensure by the Commonwealth of Massachusetts as a Licensed Practical Nurse (LPN).

• Acceptable screening against the OIG-LEIE (Office of the Inspector General’s List of Excluded Individuals and Entities).
• Valid driver’s license with the ability to transport individuals in an agency vehicle and willingness to use a personal vehicle for business purposes.
• Must meet agency driver requirements, including annual RMV checks and proof of physical capability to assist participants with transfers and mobility equipment.
• Documentation of a recent Tuberculosis (TB) screening test (required every two years); Hepatitis B immunization recommended.

ESSENTIAL DUTIES
• Support the Director of Specialized Services in managing day-to-day operations, including medical appointments, hospital discharges, and transitional care.
• Monitor the general health status of participants and respond appropriately to medical concerns.
• Attend and participate in ISP and transition meetings as needed.
• Coordinate and implement service plans, ensuring medical, behavioral, and therapeutic supports are followed.
• Assist with case management and required health documentation.
• Facilitate staff meetings and ensure staff are trained in relevant health and safety procedures.
• Communicate with LPNs, physicians, pharmacists, and other medical providers to ensure continuity of care.
• Administer and document medications, treatments, and first aid as needed.

CERTIFIED NURSING ASSISTANT DIRECT CARE SUPPORT - ABI
The CNA/ABI is responsible for providing an array of services to enrich the lives of people with Acquired Brain Injury (ABI). Various levels of support are provided in a number of areas, including but not limited to personal care, life routines, finances, health care and clinical services, recreational and social interests, developing relationships, and connecting to the community.

All duties shall be performed in a dignified and supportive manner so as not to diminish a person's self-identity or self-esteem. The ideal candidate will be experienced in providing diverse care to those who need it while remaining patient and calm.

DIRECT SUPERVISOR:
Residential Coordinator and/or Community Living Supervisor

A Sampling of Essential Duties:
• Enhance individuals' lives by monitoring needs and offering personalized assistance and support with activities of daily living to improve quality of life.
• Assess and report any issues to other healthcare personnel.
• Supervise, support, and maintain the safety of assigned individuals.
• Provide support, training, and assistance to individuals as assigned by the supervisor. These areas may include personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP and as needed in life routines.
• Participate as a team member in the ISP process and in person-centered planning as applicable.
• Implement support plans throughout the day, including behavioral and therapeutic requirements.
• Provide case management responsibilities and/or service documentation requirements as assigned and defined by the supervisor.
• Attend meetings, as required, to facilitate participant progress, Agency growth, or general communication among staff and for other agencies.
• Provide training, support and assistance as necessary to aid people in adapting to and participating in community environments.
• Attend all required training and maintain required certifications.
• Attend work on a regular, consistent basis.
• Provide or facilitate transportation arrangements reinforcing the use of natural supports when possible.
• Perform all duties in conformance to, or in recognition of, existing Agency philosophy, policies, and procedures.
• Assist individuals with medical appointments as necessary.
• Administer and document medication per DDS regulations.
• Ensure all required documentation is completed, including, but not limited to, progress notes, program data, medication, cleaning checklist, daily log/staff communication, finances, and time slips.

This position is based in the Dighton area working Monday through Friday with flexibility. The Residential Service Coordinator is responsible for running one (1) house.

Residential Direct Care Range from $20 to $22.72 per hour with MAP / UP TO $2K SIGN ON BONUS!

Cooperative Production has 33 residential homes and 6 Day Programs. Our 24/7 supervised residential programs focus on a person-centered planning approach and working with individuals to create homes adapted to meet individual needs. We provide support in the areas of life skills, social skills, and community integration with an emphasis on pursuing personal goals and interests. Each day our staff provides guidance, support, and direction as needed on new skills/tasks, behavior management, personal development, social skills, and communication. They take pride in assisting individuals in achieving goals by maintaining knowledge of their learning styles, strengths, and support needs and ensuring that skills are taught accordingly.

RESIDENTIAL DIRECT CARE
Provide support, training, and assistance to individuals as assigned by the supervisor. These areas may include personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP and as needed in life routines. Participate as a team member in the ISP process and in person-centered planning as applicable. Implement support plans throughout the day, including behavioral and therapeutic requirements. Ensure all required documentation is completed, which includes but is not limited to progress notes; program data; medication; cleaning checklist, daily log/staff communication; finances; and time slips. Previous experience working with individuals with intellectual and/or physical disabilities is preferred; however, training is provided for those who do not have the experience but who have the heart for this work. Daily administering and documenting the medications of each individual as prescribed by a physician in accordance with the Medication Administration Policy (MAP) is a must.

Requirements:
• Possession of a valid driver’s license, a safe driving record, and an agreement to use one’s personal vehicle for business purposes.
• Acceptable Criminal Offender Record Information (CORI) & Fingerprinting required
• MAP certification is required within three months of hire date - Higher pay rate is based on being MAP certified

WORKING CONDITIONS
Employees are required to assist participants who may demonstrate aggressive or self-injurious behavior through direct intervention and conformance with behavioral programs.
Employee assignments and work schedules are subject to change contingent upon the needs of individual participants or groups of participants.

Residential Community Living Supervisor Range from $23.25 to $24.57 per hour / UP TO $2K SIGN ON BONUS

The Community Living Supervisor is responsible for supervising and implementing an array of services to enrich the lives of people with developmental disabilities. Services include but are not limited to, case management, transportation, the development of social relationships, and providing support in the areas of recreational, therapeutic, and personal living skills.

Duties shall be carried out in conformance to the Agency’s purpose and philosophy and in a manner that recognizes values associated with Life Quality.

This position is scheduled Saturday 9:00 am - 5:00 pm, and Tuesday through Friday 1:00 pm - 9:00 pm.

QUALIFICATIONS
• High School degree required, college degree preferred.
• Ability to comprehend and write English at the traditional Grade 12 level.
• Possession of a valid driver’s license and an agreement to use one’s personal vehicle for business purposes.
• Three years of management experience working with people with developmental disabilities.
• Acceptable Criminal Offender Record Information (CORI) and Fingerprint check.

ESSENTIAL DUTIES
• Oversee the day-to-day operation of the program (staff scheduling, medical appointments, community activities, maintenance, vehicles, menu planning, etc.
• Trains, supervises, and evaluates residential staff in conjunction with the C.O.C.L./Residential Coordinator II. Participates in interviewing and hiring new staff.
• Supervise, support, and maintain the safety of individuals.
• Provide relevant training or assistance to individuals as identified in their ISP and/or as needed in life routines. Specific “training” may be in: communication skills, personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP.
• Monitor and implement service plans throughout the day including behavioral and therapeutic requirements.
• Provide case management responsibilities and/or service documentation requirements as assigned and defined by the C.O.C.L./Residential Coordinator II.
• Facilitate monthly staff and house meetings.
• Provide counseling and assistance as necessary to aid people in adapting to a new workplace or other community environments.
• Attend required In-services.
• Provide or facilitate transportation arrangements optimally using natural supports.
• Perform all duties in conformance to, or in recognition of existing Agency policies and procedures including the personnel Manual.
• Assist individuals with medical appointments as necessary.
• Monitor individual’s medical needs as well as doctor’s orders, medication sheets, diets, etc.
• Ensure required documentation is completed including but not limited to Progress Notes; program data; medication; cleaning checklist; Daily Log/Staff Communication; finances; time slips, etc.
• Assume various other and related duties as assigned by the C.O.C.L./Residential Coordinator II.
• Where necessary, serve as a communication link between residents, family members, guardians, and other service providers.
• Assist in the management of the program budget and financial record keeping to ensure accuracy and compliance.
