The RN MAP Trainer is responsible for designing, delivering, assessing, and continually improving the agency’s Medication Administration Program (MAP) for staff, volunteers, and interns. This position ensures compliance with all state and federal regulations, accreditation standards, and organizational policies related to medication administration. The RN MAP Trainer combines instructional leadership, hands-on competency evaluation, coaching, and quality monitoring to ensure safe and accurate medication practices across residential and day services.
This role works closely with nursing, clinical, and administrative leadership to maintain high standards of medication safety and regulatory compliance throughout the organization.
DIRECT SUPERVISOR

QUALIFICATIONS
• Current licensure by the Commonwealth of Massachusetts as a Registered Nurse (RN).

WORK SCHEDULE & ENVIRONMENT

ESSENTIAL DUTIES
• Develop, implement, and maintain MAP training curriculum aligned with regulatory standards.
• Deliver instruction using varied teaching methods, including classroom, simulation, e-learning, and hands-on practice.
• Conduct prerequisite screenings, competency assessments, and skills validations for MAP participants.
• Create and administer written and practical examinations, including scenario-based evaluations.
• Maintain current MAP policies, procedures, and training materials.
• Provide coaching and remediation for staff requiring additional competency support.
• Facilitate new employee MAP onboarding and annual recertification processes.
• Track certification status, expiration dates, pass/fail rates, and remediation metrics.
• Maintain accurate documentation within the agency’s training database.
• Collaborate with leadership and quality assurance teams on audits, incident reviews, and quality improvement initiatives.
• Stay current with regulatory updates, medication safety best practices, and disaster preparedness protocols.
• Schedule and proctor medication administration testing within required regulatory timeframes.
• Assist with medication reviews and related compliance tracking.
• Provide on-site and remote support to programs as needed.
• Perform additional duties as assigned.

The ABI Nurse Coordinator is responsible for supporting and coordinating an array of services to enhance the health and well-being of individuals with acquired brain injuries (ABI) and other disabilities. This includes overseeing daily medical needs, coordinating care across programs, facilitating transitions between healthcare settings, and ensuring the delivery of services in a manner that promotes dignity, independence, and quality of life. The ABI Nurse Coordinator plays a critical role in maintaining communication between program staff, families, guardians, and medical professionals to ensure each individual’s needs are met.

ESSENTIAL DUTIES
• Support the Director of Specialized Services in managing day-to-day operations, including medical appointments, hospital discharges, and transitional care.
• Monitor the general health status of participants and respond appropriately to medical concerns.
• Attend and participate in ISP and transition meetings as needed.
• Coordinate and implement service plans, ensuring medical, behavioral, and therapeutic supports are followed.
• Assist with case management and required health documentation.
• Facilitate staff meetings and ensure staff are trained in relevant health and safety procedures.
• Communicate with LPNs, physicians, pharmacists, and other medical providers to ensure continuity of care.
• Administer and document medications, treatments, and first aid as needed.
• Ensure medication storage, disposal, and documentation practices comply with regulations.
• Monitor and report on health-related data, including hygiene, skin integrity, seizure records, and diet compliance.
• Participate in interviewing and hiring of LPN staff.
• Schedule and coordinate nursing coverage and on-call responsibilities.
• Maintain effective communication with families, guardians, and residential staff regarding health issues.
• Attend and conduct required in-services and trainings.

1. Assist the COO and other senior managers in the development, implementation, and monitoring of operational practices, policies, and procedures, in coordinating services across programs, and in general administrative activities/functions.
2. Ensure that individuals’ rights are protected, and promote opportunities for individuals to exercise control, decision-making and choice-making.
3. Ensure that services are meaningful to people’s interests, needs and status as adults; delivered in a dignified manner; and related to established goals.
4. Keep abreast of regulatory standards and trends for service quality; assist Chief Operating Officer, Director of Residential Services (DRS), Director of Specialized Services (DSS), and Director of Day Services (DDS) in preparing for service audits including Licensing & Certification, CARF, and Mass Health/LTSS.
5. Promote quality services by enforcing established policies, procedures and regulatory standards related to quality assurance.
6. Interact with individuals/families to identify opportunities to improve service quality by conducting annual Satisfaction Surveys and charting results for Annual Management Report.
7. Assist COO in establishing quality standards by reviewing regulatory requirements; devising best practice methods and procedures, and facilitating collection of data and metrics to measure quality of programs and success of systems.
8. In coordination with COO, perform root-cause analysis and other problem-solving activities to identify effective corrective actions and process improvements related to service quality. Identify both problems and solutions; and provide recommendations.
9. In coordination with COO, develop and implement system for internal quality assurance audits; serve as primary resource for problem identification, resolution and continuous service quality improvement. As needed, assist with development of new systems to improve quality of services and/or data collection.
10. Implement program quality assurance activities as appropriate to raise level of service quality.
11. Monitor compliance with state and federal regulatory requirements and licensing/accreditation standards across all agency programs.
12. Ensure services conform with the highest standards of health and safety. Coordinate the review and compliance with person served medication administration, protocols, physician orders and appointment follow-ups.
13. Act as a MAP trainer in the absence of MAP coordinators
14. Ensure all Fire Drills and Safety Plans meet DDS requirements, and complete/update and submit EESPs in conformance to regulatory mandates, in coordination with DRS/DSS.
15. Maintain system for reviewing and tracking financial records for residents and assist as needed with financial audits.
16. Oversee tracking system for ensuring conformance to regulatory timelines related to ISP submissions, ensuring on-time submissions of required documents (assessments, support strategies, etc.).
17. Monitor HCSIS Alerts/ Process Management to ensure conformance to regulations and to address required Agency follow-up.
18. Ensure conformance to Agency philosophy, policies and procedures within assigned programs/services.
19. Oversee the property management database system and provide training and support as needed, to ensure that facilities and vehicles are properly maintained.
20. Assist in public relations efforts; represent agency as appropriate, participate in area committees as appropriate.
21. Ensure completion of required documentation as requested by COO or Senior Managers and maintain/improve all related forms and tracking systems.
22. Work with COO to complete Accessibility Plan, Risk Management Report, and other reports or tasks as required.
23. Maintain and improve systems for tracking referral, intake, transfer, and discharge activities.
24. Implement and monitor an Electronic Healthcare Record database across all interdisciplinary departments.

CERTIFIED NURSING ASSISTANT DIRECT CARE SUPPORT - ABI
The CNA/ABI is responsible for providing an array of services to enrich the lives of people with Acquired Brain Injury (ABI). Various levels of support are provided in a number of areas, including but not limited to personal care, life routines, finances, health care and clinical services, recreational and social interests, developing relationships, and connecting to the community.
All duties shall be performed in a dignified and supportive manner so as not to diminish a person's self-identity or self-esteem. The ideal candidate will be experienced in providing diverse care to those who need it while remaining patient and calm.
DIRECT SUPERVISOR:
Residential Coordinator and/or Community Living Supervisor
A Sampling of Essential Duties:
• Enhance individuals' lives by monitoring needs and offering personalized assistance and support with activities of daily living to improve quality of life.
• Assess and report any issues to other healthcare personnel.
• Supervise, support, and maintain the safety of assigned individuals.
• Provide support, training, and assistance to individuals as assigned by the supervisor. These areas may include personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP and as needed in life routines.
• Participate as a team member in the ISP process and in person-centered planning as applicable.
• Implement support plans throughout the day, including behavioral and therapeutic requirements.
• Provide case management responsibilities and/or service documentation requirements as assigned and defined by the supervisor.
• Attend meetings, as required, to facilitate participant progress, Agency growth, or general communication among staff and for other agencies.
• Provide training, support and assistance as necessary to aid people in adapting to and participating in community environments.
• Attend all required training and maintain required certifications.
• Attend work on a regular, consistent basis.
• Provide or facilitate transportation arrangements reinforcing the use of natural supports when possible.

CERTIFIED NURSING ASSISTANT DIRECT CARE SUPPORT - RESIDENTIAL
The CNA/Residential is responsible for providing an array of services to enrich the lives of people with intellectual and developmental disabilities, as well as physical disabilities. Various levels of support are provided in a number of areas, including but not limited to personal care, life routines, finances, health care and clinical services, recreational and social interests, developing relationships, and connecting to the community.
All duties shall be performed in a dignified and supportive manner so as not to diminish a person's self-identity or self-esteem. The ideal candidate will be experienced in providing diverse care to those who need it while remaining patient and calm.

A Sampling of Essential Duties:

This position is based in the Dighton area working Monday through Friday with flexibility. The Residential Service Coordinator is responsible for running one (1) house.

• Provide case management responsibilities and/or service documentation requirements assigned and defined by the Residential Services Director.
• Assume supervision responsibility for assigned staff, which may include nursing staff.
• Train, assist, and evaluate staff in relevant areas of their job description; initiate disciplinary procedures with the Residential Services Director.
• Assume “on-call” responsibilities as assigned.
• Where necessary, serve as a communication link between residents and family members. Assume direct responsibility for the management and protection of agency and resident funds.
• Ensure that residents are involved in the development of menus and in scheduling their other shared responsibilities as members of the household.
• Ensure regularly scheduled house meetings with residents are conducted.
• Ensure the home is well maintained both inside and out.
• Assume duties of residential staff as necessary.
• Perform all duties in conformance to and/or in recognition of existing agency philosophy, policies, procedures, and DMR regulations.
• Oversee the coordination and follow-up of all necessary medical appointments.
• Administer medications and ensure medications are administered in accordance with DDS/DPH regulations.
• Develop staff schedules and assure adequate coverage.

Our 24/7 supervised residential programs focus on a person-centered planning approach and working with individuals to create homes adapted to meet individual needs. We provide support in the areas of life skills, social skills, and community integration with an emphasis on pursuing personal goals and interests. Each day our staff provides guidance, support, and direction as needed on new skills/tasks, behavior management, personal development, social skills, and communication. They take pride in assisting individuals in achieving goals by maintaining knowledge of their learning styles, strengths, and support needs and ensuring that skills are taught accordingly.
RESIDENTIAL DIRECT CARE
Provide support, training, and assistance to individuals as assigned by the supervisor. These areas may include personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP and as needed in life routines. Participate as a team member in the ISP process and in person-centered planning as applicable. Implement support plans throughout the day, including behavioral and therapeutic requirements. Ensure all required documentation is completed, which includes but is not limited to progress notes; program data; medication; cleaning checklist, daily log/staff communication; finances; and time slips. Previous experience working with individuals with intellectual and/or physical disabilities is preferred; however, training is provided for those who do not have the experience but who have the heart for this work. Daily administering and documenting the medications of each individual as prescribed by a physician in accordance with the Medication Administration Policy (MAP) is a must.
Requirements:
• Possession of a valid driver’s license, a safe driving record, and an agreement to use one’s personal vehicle for business purposes.
• Acceptable Criminal Offender Record Information (CORI) & Fingerprinting required
• MAP certification is required within three months of hire date - Higher pay rate is based on being MAP certified

The Community Living Supervisor is responsible for supervising and implementing an array of services to enrich the lives of people with developmental disabilities. Services include but are not limited to, case management, transportation, the development of social relationships, and providing support in the areas of recreational, therapeutic, and personal living skills.
Duties shall be carried out in conformance to the Agency’s purpose and philosophy and in a manner that recognizes values associated with Life Quality.
This position is scheduled Saturday 9:00 am - 5:00 pm, and Tuesday through Friday 1:00 pm - 9:00 pm.
QUALIFICATIONS
• High School degree required, college degree preferred.
• Ability to comprehend and write English at the traditional Grade 12 level.
• Possession of a valid driver’s license and an agreement to use one’s personal vehicle for business purposes.
• Three years of management experience working with people with developmental disabilities.
• Acceptable Criminal Offender Record Information (CORI) and Fingerprint check.
ESSENTIAL DUTIES
• Oversee the day-to-day operation of the program (staff scheduling, medical appointments, community activities, maintenance, vehicles, menu planning, etc.
• Trains, supervises, and evaluates residential staff in conjunction with the C.O.C.L./Residential Coordinator II. Participates in interviewing and hiring new staff.
• Supervise, support, and maintain the safety of individuals.
• Provide relevant training or assistance to individuals as identified in their ISP and/or as needed in life routines. Specific “training” may be in: communication skills, personal hygiene, daily living skills, recreation, and/or other skills as determined by the individual’s ISP.
• Monitor and implement service plans throughout the day including behavioral and therapeutic requirements.
• Provide case management responsibilities and/or service documentation requirements as assigned and defined by the C.O.C.L./Residential Coordinator II.
• Facilitate monthly staff and house meetings.
• Provide counseling and assistance as necessary to aid people in adapting to a new workplace or other community environments.
• Attend required In-services.
• Provide or facilitate transportation arrangements optimally using natural supports.
• Perform all duties in conformance to, or in recognition of existing Agency policies and procedures including the personnel Manual.
• Assist individuals with medical appointments as necessary.
• Monitor individual’s medical needs as well as doctor’s orders, medication sheets, diets, etc.
• Ensure required documentation is completed including but not limited to Progress Notes; program data; medication; cleaning checklist; Daily Log/Staff Communication; finances; time slips, etc.
• Assume various other and related duties as assigned by the C.O.C.L./Residential Coordinator II.
• Where necessary, serve as a communication link between residents, family members, guardians, and other service providers.
• Assist in the management of the program budget and financial record keeping to ensure accuracy and compliance.
